Nothing

If your tama is sick or has a toothache, that could be part of the problem, but it would also be obvious because your tama would look sad and have either a skull or a tooth icon floating above its head. In that case, use the B button on the medicine bag until it is healed.

Sounds to me, like Courtnee said, that your happy hearts are drained. To fix this, feed your tama a snack to get a happy heart back, or play a game until you get at least a "Good!" rating. For each "Good!" or above game rating, you'll get 1 filled happy heart.
