Yes, Necktietchi is obtained from bad care. Keep trying different ways to care for him and they'll evolve into different adults.

 
I am trying to evolve my Hoshitchi into that moon character. I forgot the name, but i'll try to evolve it into Chamametchi or Makiko.

 
Remember to keep playing games with your Tama even if the happiness hearts are full and the weight is healthy. That way, you increase the friendship level, which lowers your chance of getting a horrible care character.
