I'm guessing the teen on the far left is perfect care and this chart ranges from perfect care on the left to horrible care on the right. I can't read it either, but I'm assuming the snail teen is the worst care teen.

 
But what does the 20 bit mean? I can guess the 4-5 etc... But that's one thing I don't get
