So for example if the POTM is the cow and i already have the cow will i have the POTM items or do i have to adopt a new one during its POTM time?

If you already have the POTM before it is POTM, all you will be able to do with your pet is to do activities that only users with POTM can do. If you want the loot bag, you'll have to buy another.

Oh okay thanks but one more question. Will the lil' kinz versions of the POTM be able to be POTM

No. You're Lil' Kinz Googles will not work. You need a full-sized Googles to participate in POTM activities and get a POTM loot bag if registered next month.
