Sometimes it happens when you pause your tamagotchi a lot. That happened to mine when it was 2 and it was a Kuchipatchi!

Don't worry, your tama just put on a costume. It will stay on for a day, then go away, but if you want to keep it, pause your tama when it sleeps.
